I don't know about all shows, but for ARBA shows, it means the show points, winnings, etc are kept on record by the ARBA for it's members.

Even at an ARBA sanctioned show, some breeds may not be sanctioned. These are normally breeds that are not yet accepted into the standard, or breeds that will not have enough exhibitors to make it worthwhile for the club to pay the sanction fee.
 
A sanction is issued when a local club which is hosting a show pays fees recognizing the breed as commonly shown at their shows, and the sanction itself allows breeders who show there and are members of the national club, to receive points in their national sweepstakes contests. The sanctions are usually paid as an added incentive to draw in exhibitors of various breeds.

Several state associations also have sanctioning for local shows, and it allows the same.
 
Additionally, if your breed is not sanctioned, you can request the show secretary to sanction the breed. It may mean you will need to pay the sanction instead of it being paid by the club hosting the show. I believe the sanction fee is $10.
 
Sanctions start at the national ARBA club level. Without an ARBA sanction no legs can be issued. Once the ARBA sanction is received then breed club sanctions can be done. Fees do vary for each club. After the show the show secretary will send the placings for breed clubs that were sanctioned and breeders that are members of that breed club will get points towards sweepstakes.
